Your full name, phone number, optional email address and the short note you write are stored. ID documents, test results and a detailed medical history are not required for this form.

The team uses this information to respond to your request and arrange your appointment time. Submitting the form does not automatically confirm an appointment.

Storage and protection

Requests are not displayed on public pages. Access to them is provided through a protected admin panel. New request data is stored on the server in encrypted form and protected by backups.

Third-party services

When you open WhatsApp, social media or map links, the privacy policies of those services apply. The embedded map loads only when you open it. A strictly necessary session cookie is used for admin login; no advertising trackers have been added to the site.

Data requests

You can contact the team to have your information corrected or deleted. If you have separate questions about retention periods or medical records, please ask the reception team.
